***Students planning to bring a non-CB South guest or a CB South guest (in a different grade) to any school sponsored dance/prom need to have a guest form approved. Students may not buy a guest ticket without this form.
2026 JUNIOR Prom - Student Guest Form
This form is to be used to bring any non-CB South guest to a CB South sponsored dance. Guests must be in at least 10th grade and are not to be more than 21 years old. Part of this form requires a signature from the guest's school, so you should prepare for this before ticket sales begin. All forms should be turned into the 2nd Floor House Office for approval by your House Principal. A picture ID (a copy) of the guest must be submitted with the form for approval. Any questions should be directed to your house office.
