Dance Guest Form

                          

        

 

 

 

This form is to be used to bring any non CB South guest to a CB South sponsored dance. Guests must be in at least 10th grade and are not to be more than 20 years old. Part of this form requires a signature from the guests school so you should prepare for this before ticket sales begin. All forms should be turned into your house office for approval by your house principal. A picture ID (a copy) of the guest must be submitted with the form for approval. Any questions should be directed to your house office.
